The NHS Condition Management Programme (CMP) is delivered by a multi-disciplinary healthcare team comprising Occupational Therapists, Physiotherapists and Mental Health Practitioners.
The CMP is accessible in community settings across the North Staffordshire, Stoke-on-Trent and South Staffordshire localities.
The service can support people with many long term conditions including:-
- Mental health conditions
- Musculo-skeletal conditions
- Cardio-respiratory conditions
The CMP offers advice, education and reassurance to people to enable them to manage their health conditions more effectively.
The Programme further aims to:-
- Provide people of working age with health problems the same employment opportunities as everyone else
- Help people break down barriers which prevent them getting back to work
- Help people with a moderate health condition or disability, who have clear potential, to return to work
If an individual is in receipt of any of the following benefits they may be eligible for referral to the CMP:
- Employment Support Allowance (ESA)
- Incapacity Benefit (including credits only)
- Income Support (because of their incapacity)
- Severe Disablement Allowance
