Will being on the Programme affect my benefits ?
No. Participation on the programme is purely voluntary. If at the end of the programme you are able to consider returning to work, there are additional benefits that will be available to you both financially and personally.
How long does the Programme last ?
Approximately 16 weeks.
How can I take part ?
You will need to contact your Personal Advisor at your local Jobcentre Plus office who will refer you to the Programme.
Will the information I provide to my CMP Case Manager be shared with the Jobcentre?
The Condition Management Programme is an NHS service and therefore NHS standards of confidentiality and compliance with the Data Protection Act apply. Information will not be shared with another person unless we have your express consent to do so.
Where is it?
The first meeting with a CMP Case Manager takes place at the customer’s local Jobcentre Plus office. After that, CMP services are delivered from various community locations across the county and as close to people’s home localities as possible.
Can CMP get me a job?
Staffordshire CMP is an NHS health service which supports individuals in managing their long term health condition in order to consider returning to work. Part of this process includes raising self esteem, confidence building, assertiveness training, etc., all of which may help an individual in their ability to gain employment. However, contacting potential employers and support with CV writing and interview attendance, etc., is carried out by our partner organization, Jobcentre Plus. The customer’s CMP Case Manager and Personal Advisor at the Jobcentre, work together to provide the best opportunities for the customer where appropriate.
Can I still see my GP/Nurse/counsellor?
Yes – if you are referred to the CMP but are also receiving health services from other NHS staff, you will need to continue your treatment. The CMP will notify your GP, and any other health professional you may be receiving care from, that you are attending the CMP service.
What happens if I find a job during my time with the CMP?
You will be able to complete the planned activities you have started on the programme, if your new working hours allow you to do so.
Will my name be published anywhere if I take part in the programme?
No, all customer information is strictly confidential and is not shared with other organisations or individuals without the express consent of the customer concerned. Customers are asked to complete evaluation forms, and occasionally evaluation findings are published, but data is always anonymised to protect the identity of those customers who have contributed to this work.
Can I claim my travel expenses for attending the service?
Reasonable travel expenses can be claimed for journeys made by customers to attend CMP services.
